Please sign the following waivers to participate
Please complete your waiver prior to visiting Titletown for Fitness Activities programs. Please note:
- Each adult (18 and older) must complete his/her own waiver.
- Each adult may add up to 10 minors per waiver.
- You will receive a confirmation email upon successful completion of the online waiver.
- Your completed waiver is valid through December 31, 2023.
